Forum OpenACS Q&A: Has anybody created a multiple choice form element widget with an "other" field?

Has anybody created a multiple choice form element widget with an "other" field? By this i means something like this where the "o"'s are radios:

Pick your favorite color

o Red
o Green
o Blue
O Other: ____________________

Ultimately i would like a checkbox and radio widgets with an other field. If you have created this and if you don't mind email the code to me i would appreciate it. If nobody has done this i'll write a template element to do it.
